Job Description:
A company is looking for a Licensed Clinician to provide
behavioral health treatment in primary care settings.
Key Responsibilities
Provide evidence-based interventions using the Collaborative Care
Model
Collaborate with primary care providers and psychiatric consultants
to optimize patient outcomes
Utilize technology platforms to deliver brief interventions and
behavioral health screenings
Required Qualifications and Education
Master's Degree in Social Work, Counseling, or a related field
Licensed to practice as an independent clinician in Connecticut
(LCSW/LICSW, LMHC, LMFT, LPC, or equivalent)
Proficiency in evidence-based treatment approaches
Experience with higher-acuity patients and behavioral health
screening tools
Technical proficiency in EHRs and ability to learn new technology
quickly
